Job summary

Hospital to Home LLC in Charlottesville, VA is seeking a Paramedic to join our EMS team. You will provide advanced medical care in the field, handle interfacility transfers and in-home assessments, and support transitions from hospital to home.

You will operate EMS equipment, document care accurately, educate patients and families, maintain licensures, and collaborate with RNs and case managers to ensure safe, timely patient care.

Qualifications

• High School Diploma or GED required; Associate’s Degree in EMS or related field preferred.
• Current State Paramedic License (and NREMT if required by state).
• Current certifications: BLS, ACLS, PALS; ITLS/PHTLS preferred.
• Valid driver’s license for more than 3 years and acceptable driving record.
• Must be 21 years of age or older.
• Must be able to lift 125 lbs. individually and 250 lbs. assisted.

Responsibilities

• Respond to calls for medical assistance, interfacility transfers, and in-home assessments.
• Provide advanced life support (ALS) care including medication administration, airway management, cardiac monitoring, and trauma stabilization.
• Collaborate with RNs, case managers, and other healthcare professionals to support seamless patient transitions from hospital to home.
• Monitor and document patient condition during transport, communicating any changes to medical personnel and receiving facilities.
• Operate and maintain medical equipment and the ambulance in a state of readiness.
• Ensure timely, accurate, and complete documentation of all patient care provided.
• Educate patients and families on medications, follow-up care, and health management when applicable.
• Uphold patient confidentiality and professional standards in all interactions.
