Hospital to Home is hiring a Paramedic to provide advanced medical care in both emergency and non-emergency transport scenarios. You will work to bridge hospital care and home health, delivering safe, efficient transport and supportive coordination with care teams.

The role requires state paramedic licensure and CPR certifications, with shifts that may include nights, weekends, and holidays. You will educate patients on medications and care plans while maintaining high professional standards.

Qualifications

• High School Diploma or GED required; Associate’s Degree in EMS or related field preferred.
• Current State Paramedic License (and NREMT if required by state).
• Current certifications: BLS, ACLS, PALS; ITLS/PHTLS preferred.
• Valid driver’s license for more than 3 years and acceptable driving record.
• OEMS Paramedic certification or equivalent preferred.
• Strong interpersonal and clinical assessment skills.
• Prior EMS/mobile healthcare or community paramedicine experience is a plus.

Responsibilities

• Respond to calls for medical assistance and interfacility transfers.
• Provide advanced life support (ALS) including meds, airway, cardiac monitoring.
• Collaborate with RNs, case managers and other professionals for patient transitions.
• Monitor and document patient condition during transport and communicate changes.
• Operate and maintain ambulance and medical equipment.
• Ensure timely, accurate documentation of care provided.
• Educate patients and families on medications and follow-up care.

In this role, you will provide advanced medical care in the field, both in emergency and non-emergency settings.
