Job summary

Hospital to Home LLC is seeking a skilled Paramedic to join our patient-centered medical transport team. You will deliver advanced life support and manage medical care during emergency and non-emergency transports, bridging hospital and home health.

You will work variable shifts, including nights and weekends, and collaborate with RNs, case managers, and other clinicians to ensure safe, efficient patient transitions and accurate documentation.

Qualifications

• High School Diploma or GED required; Associate’s Degree in EMS or related field preferred.
• Current State Paramedic License and NREMT if state requires it.
• Current BLS, ACLS, PALS; ITLS/PHTLS preferred.
• Valid driver’s license for >3 years and clean driving record.
• EVOC certification or equivalent preferred.
• Strong interpersonal and clinical assessment skills.
• Must be 21+ years old; able to lift 125 lbs. individually and 250 lbs. assisted.
• Must maintain OEMS Paramedic certification.

Responsibilities

• Respond to calls for medical assistance, interfacility transfers, and in-home assessments.
• Provide ALS care including medication administration, airway management, cardiac monitoring, and trauma stabilization.
• Collaborate with RNs, case managers, and other healthcare professionals to support patient transitions.
• Monitor and document patient condition during transport and report changes to medical staff.
• Operate and maintain ambulance and medical equipment in ready state.
• Ensure timely, accurate documentation of all patient care provided.
• Educate patients and families on medications, follow-up care, and health management when applicable.
• Uphold patient confidentiality and professional standards in all interactions.
