Job summary

LivWell Community Health Services in Hopkinsville, KY is seeking a Support Services Amenities Specialist to coordinate food pantry services, transportation, and related documentation. You will maintain records, schedule transport, prepare pantry orders, and ensure clients have access to essential health and support services.

Qualifications

• High School Diploma is required.
• Minimum 2 years in grocery/retail/transport/public service preferred.
• Valid driver's license and safe driving record required.

Responsibilities

• Maintain accurate and uptodate forms for client orders.
• Collect and organize food pantry order forms with client signatures.
• Complete case notes and documentation in CAREWare for pantry and transportation services.
• Schedule and organize transportation services for clients.
• Verify all transportation documentation is complete (transportation request, vehicle use request, gas card).
• Coordinate and assist with packing of food pantry orders for distribution.
• Greet and distribute food pantry orders to clients.
• Update pantry and transportation lists with Case Management Team.
