Job summary

LivWell Community Health Services is seeking a Support Services Case Manager in Hopkinsville, KY. The role focuses on guiding clients through social services and healthcare systems, coordinating resources, setting goals, and ensuring continuity of care with a multidisciplinary team.

The position involves assessing needs, developing care plans, and assisting with Medicaid/Medicare documentation while traveling for training, in-home visits, and clinics as required.

Qualifications

• Bachelor’s degree in nursing, social work, counseling, psychology, human services OR Kentucky state license as LPN OR ADN.
• Knowledge of community resources and sensitivity towards people living with HIV/AIDS.
• Valid driver’s license and are insurable to drive LivWell CHS' vehicles.
• Experience with basic technology and Microsoft Office Suite.

Responsibilities

• Meeting with clients to assess needs, verify eligibility for services, and introduce opportunities within the program for assistance.
• Developing an Individualized Care Plan in collaboration with the client to secure suitable support services.
• Determining and providing the level of assistance and referring emergency needs to the appropriate service area.
• Coordinating care plans with area resources, transportation to appointments, education, advocacy, and linking to external agencies.
