Job summary

By The Bay Health seeks a Palliative Care Coordinator to provide administrative support to the Palliative Care and Pediatric teams. The role will operate under BTBH policies and applicable laws, ensuring accurate Epic reporting and coordinated care delivery.

Schedule is Monday–Friday, 8:30am–5:00pm, onsite in Larkspur, CA with occasional coverage at our Sonoma office. The position requires an AA degree or equivalent and at least two years of relevant administrative experience in a mid-to-senior

Qualifications

• AA degree or equivalent; two years related experience.
• Minimum two years’ experience providing administrative support to a mid-level or senior manager.

Responsibilities

• Run EPIC reports to collect and track metrics in alignment with program goals.
• Perform MATCH audits and outreach to MATCH patients.
• Coordinate and schedule clinician visits based on contracts and Epic reporting.
• Partner with vendors, hospitals, and facilities to facilitate strong referral pathways and educate about programs.
