Job summary
Anew Palliative Care LLC is seeking a qualified Palliative Care Nurse Practitioner to provide comprehensive home care in Indiana under physician supervision. This role focuses on symptom management, care planning, prescribing within licensure, and collaborating with an interdisciplinary team to support patients and families in the community.

Qualifications
- Master's degree with NP experience in palliative care and/or hospice
- Current nursing licensure in Indiana and CPR certification
- Certification in a specialist area preferred (e.g. CHPN)
- Excellent observation, verbal and written communication skills
- Ability to lift and transfer patients as required
- Valid driver's license with insured vehicle
Responsibilities
- Provides medical care and manages terminal illness according to Nurse Practice Act
- Completes history and physical exams and documents findings
- Develops and implements plan of care, prescribes medications as permitted
- Collaborates with interdisciplinary team to coordinate hospice care
- Reviews clinical information and oversees patient care in consultation
- Ensures symptom control through team collaboration
