Anew Hospice - Lafayette in Lafayette, IN is seeking a Palliative Care Nurse Practitioner (NP) to provide comprehensive home-based pallia tive care under physician supervision. The role includes evaluation, symptom management, and coordination with families and the interdisciplinary team in patients' homes and communities.

Qualifications

• Master's degree with NP preparation and state licensure.
• CPR certification as required by policy.
• Experience or training in palliative/hospice care preferred.
• Pediatric experience or training desirable.

Responsibilities

• Provides home-based palliative care under physician supervision.
• Performs comprehensive assessments and develops care plans.
• Prescribes medications or treatments within licensure.
• Coordinates with families and the interdisciplinary team.
• Ensures symptom control and comfort for patients.
