Job summary

The Care Team Hospice is seeking a part-time Nurse Practitioner to provide care for terminally ill patients at home in Crown Point, Indiana, and surrounding areas. The role emphasizes autonomy, collaboration with an interdisciplinary group, and delivering compassionate hospice care.

Key duties include regular patient assessments, timely Face-to-Face visits, patient/family education, and development of care plans with IDG, while maintaining HIPAA-compliant records and on-call coverage.

Qualifications

• NP must provide hospice care at home and in facilities.
• Ability to coordinate with IDG and various care settings (SNF, NF, ICF, MR).
• On-call responsibilities and timely documentation are required.

Responsibilities

• Regularly assess and reassess nursing needs of hospice patients.
• Perform Face-to-Face visits timely.
• Provide hospice nursing services, treatments and preventive procedures.
• Initiate nursing procedures for hospice care and safety.
• Observe signs and report changes to physician/IDG.
