UMass Memorial Health is seeking a licensed social worker (LICSW) to provide psychosocial support to palliative care patients and families as part of the inpatient consult team at the University campus.

The role includes assessment, counseling, discharge planning, care coordination, and collaboration with inter-disciplinary teams to ensure smooth transitions and high-quality patient care. A Master's in Social Work and LICSW licensure are required.

Qualifications

• Requires Masters in Social Work and LICSW licensure.
• Minimum of 2 years clinical experience in hospital, long-term care or hospice setting.
• Demonstrated ability in psychosocial assessment, crisis intervention, discharge planning and multidisciplinary collaboration.

Responsabilités

• Provide psychosocial support to palliative care patients and families.
• Collaborate with interdisciplinary team to develop and deliver treatment and discharge plans.
• Coordinate as inpatient social navigator and support daily operations of the consult team.
