Responsibilities

• Provide psychosocial support to palliative care patients and families in inpatient and outpatient settings.
• Participate in daily rounds and weekly team meetings, contribute to case reviews and quality improvement.
• Conduct comprehensive psychosocial assessments and discharge planning in collaboration with the inter-disciplinary team.
• Teach students, residents and fellows in palliative care concepts during rounds and didactics.
• Coordinate discharge planning and act as liaison with social service, care coordination and community services.
