Qualifikationen

• MSW from an accredited institution (degree verification required).
• Current state licensure (CSW/LCSW) prior to or upon completion of required supervision hours.
• Basic computer proficiency, including familiarity with word processing and spreadsheets.
• Strong written and verbal communication skills.
• Demonstrated ability to apply critical thinking skills.
• Caregivers who conduct home or community visits must maintain BLS, a valid driver’s license, current auto insurance, and reliable transportation.

Aufgaben

• Assess and screen patients for transition planning, mental health, substance use, and goals of care.
• Develop and monitor care plans addressing social determinants of health and community resources.
• Provide behavioral health support using motivational interviewing and therapeutic techniques.
• Deliver brief individual, group, and family therapy and psychosocial assessments.
• Identify mental, emotional, and behavioral disorders and connect patients to services.
• Educate and advocate for patients, facilitating safe transitions of care to the community.
• Collaborate with healthcare teams, insurers, and community providers for quality care.
• Lead improvement initiatives and track key metrics to ensure policy adherence.
• Facilitate clinical goals of care discussions with patients, families, and teams.
