Job summary

Intermountain Health Logan Regional Hospital is seeking a Palliative Care Social Work Care Manager I to perform psychosocial assessments, develop care plans with the care team, and provide counseling and referrals for crisis situations. The role also includes individual and family treatment as needed and collaboration with patients, families, providers, insurers, and community resources.

Qualifications

• MSW from an accredited program (degree verification required).
• Current state licensure (CSW/LCSW) prior to or upon supervision hours completion.
• Basic computer proficiency including word processing and spreadsheet software.

Responsibilities

• Assess patients for transition planning, mental health, substance use, and goals of care.
• Develop and monitor care plans addressing social determinants of health and community resources.
• Provide brief individual, group, and family therapy; psychosocial assessments.
