Intermountain Health is seeking a Palliative Social Work Care Manager to partner with inpatient care teams to support psychosocial, emotional, and transitional needs of patients with serious illness or approaching end of life.

The role emphasizes care coordination, education, and advocacy, with focus on mental health support, safe transitions, and collaboration across healthcare and community resources in a hospital setting.

Qualifications

• Master of Social Work (MSW) from an accredited institution.
• Current state licensure if applicable (CSW/LCSW).
• Basic computer proficiency with word processing and spreadsheets.
• Strong written and verbal communication skills.
• Demonstrated critical thinking abilities.
• Drivers may require home/community visits with valid license, insurance and driving record.

Responsibilities

• Partner with inpatient Care Management and Palliative Care teams to support psychosocial needs.
• Develop and monitor care plans addressing social determinants of health and community resources.
• Provide motivational interviewing and therapeutic techniques for mental health care.
• Offer brief individual, group, and family therapy and psychosocial assessments.
• Identify mental, emotional, and behavioral disorders and connect patients to services.
• Educate patients and advocates for rights and access to care during transitions.
