Qualifications

• Requires RN licensure and clinical competence in palliative care or hospice.
• At least 2 years of RN experience as part of a palliative care or hospice team within the last 5 years.
• Familiarity with Serious Illness Conversation Guide and Vital Talk maps.

Responsabilités

• Identify ED patients for palliative care consultation in various settings.
• Perform initial patient assessments in the ED including RN Serious Illness conversations.
• Huddles with ED, Palliative Care, and other teams to assess pathway appropriateness.
• Educate clinicians on pathway criteria and patient entry into the pathway.
• Track patient progression through care pathways and disposition.
• Coordinate with Clinical Team and Case Management regarding disposition.
• Gathers clinical and social information to determine care level and location.
• Maintain documentation for the NWH ED-embedded Palliative Care Program.
• Participate in case reviews, data analysis, and quality improvement.
