Job summary

Continuum-Care-of-Washington is seeking a Registered Nurse to plan, organize, and direct hospice care in home and community settings. The role emphasizes comprehensive patient/family assessment, care planning, and education to support optimal comfort and quality of life.

The nurse will collaborate with physicians, coordinate with the interdisciplinary team, and ensure accurate documentation while maintaining a safe home-care environment.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse licensed in good standing to practice in the state of Virginia.
• Minimum of 1 year of professional nursing experience within the last 3 years (or 2 years with a BSN and relevant public health/home care experience).
• Supervisory experience not required; may supervise aides if applicable.
• Subject to background checks and references as required.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patients and families to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop, initiate and revise plans of care with patient/family involvement.
• Administer medications and treatments as prescribed and ensure safety.
• Educate patients and families on self-care and health management.
• Maintain accurate clinical notes and communicate changes with the physician.
