Job summary

Loyola Medicine Home Care and Hospice is seeking a Palliative Care RN to provide clinical consultation for patients and families in the palliative care program. This is a full-time, day-shift role with a $10,000 bonus and day-one benefits.

The RN will collaborate across departments, develop performance measures, and support quality improvement while delivering compassionate end-of-life care. Excellent teamwork and independent problem-solving are essential.

Qualifications

• Graduate of an approved nursing education program.
• Current RN licensure in the state of practice.
• CPR certification.
• 2-3 years acute care nursing and Hospice/Palliative Care certification preferred.
• Home care experience, strong clinical knowledge base, Hospice experience, computer skills, strong interpersonal and organizational skills.
• Palliative Care experience, demonstrate ability to use problem solving, critical thinking and priority setting skills.
• Must be able to work independently and on a team.
• Ability to perform diversified duties with time limitations.
• Ability to develop, coordinate and evaluate programs in Palliative care and end-of-life care.

Responsibilities

• Collaborates with manager in the development of performance measurements for the Palliative Care Program to ensure continuous quality improvement; ensures implementation.
• Motivates/supports employees for exemplary performance on internal and external customer service, productivity, and accuracy
• Provides statistical/budgetary data to appropriate Palliative care program
• Works collaboratively with all departments to ensure execution of quality delivery of services
• Ensures that stakeholders are satisfied with services provided; improvement plan developed when necessary
• Develops tracking outcomes of enrolled patients and ensures information is available for review
• A commitment to quality palliative care provision and transition to hospice when appropriate
• Exercises sound clinical judgment in oversight of Palliative Care Program
• Knows, understands, incorporates, and demonstrates the Trinity Health Mission, Vision, and Values in behaviors, practices, and decisions
