Qualifications

• RN license in practicing state; compact transfer within 60 days if applicable.
• BSN from an accredited institution; education will be verified.
• BLS certification for Healthcare Providers.
• RNs with <12 months of experience must obtain BSN within four years of hire.

Responsibilities

• Coordinate care and act as a navigator for palliative care patients and families.
• Educate patients and families about diagnosis, options, and resources.
• Assess symptoms and support management plans with providers.
• Triage calls and care concerns from patients and families.
• Coordinate services across primary care, hospice, home health, and community resources.
• Support advance care planning conversations and documentation.
• Provide ongoing follow-up and care coordination throughout the journey.
