Job summary

Intermountain Health is seeking a Palliative Care RN to join our team in Billings, MT. You will coordinate care for patients and families, educate on diagnosis and options, and collaborate with an interdisciplinary team to manage symptoms and plan care.

Qualifications

• Current RN license in the state of practice; compact-license transfer within 60 days if applicable.
• BSN from an accredited program; education verified.
• BLS for Healthcare Providers.
• RNs with less than 12 months experience must obtain BSN within four years of hire.

Responsibilities

• Coordinate care and serve as navigator for palliative care patients and families.
• Educate patients and families about diagnosis, treatment options, and resources.
• Assess symptoms and collaborate with providers on symptom management plans.
• Triage patient and family calls, questions, and care concerns.
• Coordinate services across primary care, specialty care, hospice, home health, and community resources.
• Support advance care planning discussions and documentation.
• Provide ongoing follow-up and care coordination throughout the patient journey.
