Job summary

Intermountain Health in Billings, Montana is seeking a Palliative Care RN to support patients and families with compassionate, holistic care across the care continuum. You will coordinate care, educate patients, assess symptoms, and collaborate with an interdisciplinary team; the schedule is Monday to Friday, 8:00 AM to 5:00 PM, with no routine nights, weekends, or holidays.

Qualifications

• RN license in the state of practice; compact licenses must transfer within 60 days.
• BSN from an accredited institution; education will be verified.
• Basic Life Support (BLS) certification for Healthcare Providers.
• RNs with less than 12 months of experience must obtain BSN within four years.

Responsibilities

• Coordinate care and serve as a navigator for palliative care patients and their families.
• Provide patient and family education regarding diagnosis, treatment options, and available resources.
• Assess symptoms and collaborate with providers to support symptom management plans.
• Triage patient and family calls, questions, and care concerns.
• Coordinate services across primary care, specialty care, hospice, home health, and community resources.
• Support advance care planning conversations and documentation.
• Provide ongoing follow-up and care coordination throughout the patient's healthcare journey.
