Job summary

Kaiser Permanente is seeking a skilled RN to collaborate with the interdisciplinary Palliative Care Team to coordinate clinical care for patients eligible for palliative services in inpatient or clinic settings. The role emphasizes identifying patients who may benefit, developing plans of care, and guiding transitions across care settings with a team approach.

Qualifications

• Minimum two years of experience in acute care with Medical/Surgical, Oncology and/or Critical Care Nursing or two years in Palliative Care or Hospice within the last eight years.
• Associates Degree in Nursing is required.
• Credentials from multiple states are required at hire; additional credentials post hire within 6 months.
• Basic Life Support certification required at hire.

Responsibilities

• Assess the physical, medical, psychological, social and spiritual needs of patient and family to identify goals of care and plan coordinated care across settings.
• Provide support and education to family and staff when death occurs in the inpatient setting; ensure right care at the right time.
• Participate in policy development, staff education, quality/utilization activities, and ensure regulatory compliance and professional competency.
