The RN will develop care plans, evaluate medical and psychosocial baselines, and ensure compliance with care standards. Strong collaboration with physicians and care reviewers is essential to provide continuous, high-quality patient care and discharge planning.

Responsibilities

• Plan, develop, assess, and evaluate care provided to members.
• Evaluate and develop baseline medical and psychosocial evaluations and individualized patient care/treatment plans.
• Recommend alternative levels of care and ensure compliance with federal, state, and local requirements.
• Develop individualized patient/family education plans focused on self-management.
• Encourage members to follow prescribed courses of care (e.g., drug therapy, physical therapy).
• Coordinate care/services with utilization and/or quality reviewers and monitor level and quality of care.
• Coordinate the interdisciplinary approach to providing continuity of care, including utilization management and discharge planning.
• Make referrals to appropriate community services and outside providers.
• Consult with internal and external physicians regarding continued care/treatment and hospitalization.
• Develop and maintain case management policies and procedures.
