University of Minnesota Physicians in Minneapolis seeks a Hospice and Palliative Medicine physician to provide comprehensive acute and palliative care in collaboration with physicians, APPs, social workers and chaplains. You will deliver timely consultations, ensure accurate diagnoses, and craft treatment plans with patients and families.

Qualifications

• Graduate of an accredited ACGME Hospice and Palliative Medicine training program.
• Board eligible/certified in Hospice and Palliative Medicine.
• Licensed or ability to acquire medical license and DEA in the State of Minnesota.

Responsabilités

• Collaborate with physicians, APPs, palliative care social workers and chaplains to provide timely access and care.
• Provide acute care services to patients at M Health Fairview.
• Provide timely consultations to requesting clinicians, ensure proper diagnosis and treatment.
• Communicate diagnoses, treatments, prognosis and outcomes to patients and families; coordinate with primary and referring physicians.
