HealthPartners is seeking a Certified Adult/Geriatric or Family Nurse Practitioner or Physician Assistant to join our Palliative Care team in Minnesota. The APP collaborates with physicians and other staff to deliver compassionate, state-of-the-art palliative services and to support patients and families through comprehensive assessments and care planning.

Qualifications

• Completion of a physician assistant or nurse practitioner (FNP or Adult/Gerontology) program
• Certification via appropriate discipline
• MN Licensure
• Current BLS, ACLS certification
• DEA registration

Responsibilities

• Collaborate with physicians and other medical staff to provide Palliative Care services.
• Perform comprehensive history, chart review and health assessments.
• Provide direct patient care and consultative services for state-of-the-art Palliative Care.
• Mentor, coach and support staff while carrying a patient caseload.
