Job summary

HealthDrive Corporation is seeking a compassionate Palliative Care Nurse Practitioner to provide patient-centered palliative care in skilled nursing, long-term care, and assisted living facilities.

The part-time role focuses on symptom management, goals-of-care discussions, and collaboration with residents, families, and interdisciplinary teams to improve quality of life while reducing unnecessary hospitalizations.

Qualifications

• Master’s degree in nursing from an accredited program.
• Current state NP license.
• Board certification required for practice.
• DEA/CS license and ability to obtain/maintain license renewal.

Responsibilities

• Perform comprehensive palliative care assessments for residents with serious illnesses.
• Manage symptoms such as pain, dyspnea, anxiety, insomnia, and depression.
• Develop individualized care plans with residents, families, and facility staff.
• Provide follow-up visits and monitor disease progression and treatment effectiveness.
• Order and interpret diagnostic tests and review results.
• Coordinate care with PCPs, specialists, hospice, and facility teams.
• Lead goals-of-care conversations and document advance directives.
• Prescribe medications per state policies and monitor for adverse effects.
• Educate facility staff on palliative care principles and communication strategies.
• Maintain timely, accurate EMR documentation and ensure regulatory compliance.
