MaineHealth Palliative Care is seeking an experienced Nurse Practitioner to join our Palliative Care team in MaineHealth's Mountain Region. The NP will provide inpatient consults and ambulatory care, collaborating with an interdisciplinary team in Farmington and Norway, ME.

This role requires NP master's, Maine license, BLS, and prior APP experience; base pay ranges from $119,033 to $154,043 annually, with comprehensive benefits.

Qualifikationen

• Master's degree in nursing with NP specialization.
• Current Board Certification as a Nurse Practitioner.
• Active State of Maine NP license or eligibility.
• Current BLS Certification.
• Previous APP/NP experience required.

Aufgaben

• Provides inpatient consultation services and ambulatory palliative care with the interdisciplinary team.
• Collaborates with the team to ensure quality and continuity of care.
• Adheres to evidence-based care following guidelines and best practices.
• Coordinates with primary care teams to plan goals of care and post-discharge support.
• Documents all activities timely in the EPIC EMR system.
• Participates in education of staff to promote primary palliative care.
• Respects patient confidentiality and provides timely, courteous service.
• Engages in all practice group assignments and meetings.
• Educates medical students, residents, nurses, and colleagues on palliative care.
