Job summary

MaineHealth is seeking an experienced Nurse Practitioner to join our Palliative Care team in the Mountain Region. The NP will provide inpatient consultation and ambulatory palliative care, collaborating with an interdisciplinary team to deliver patient- and family-centered care to adults with serious illness in Farmington, ME and Norway, ME.

The role emphasizes evidence-based practice, care coordination with primary teams, documentation in EPIC, and education of staff to advance primary

Qualifications

Minimum experience as an Advanced Practice Provider (APP).
Active state NP license in Maine or eligibility required.
Current BLS certification required.

Responsibilities

Provides inpatient consultation and ambulatory palliative care in collaboration with the interdisciplinary team.
Collaborates with the Palliative Care Interdisciplinary Team to ensure quality and continuity of care.
Adheres to evidence-based care following NCP guidelines.
Coordinates with primary care teams to align goals of care and post-discharge support.
Documents all activities in the EPIC EMR system.
Educates non-palliative care providers and promotes culture transformation in care delivery.
