Job summary
MaineHealth Palliative Care in Farmington, ME (with Norway, ME) seeks an experienced Nurse Practitioner to join the interdisciplinary team providing inpatient consultations and ambulatory palliative care. This full-time role collaborates with physicians, nurses, social workers and chaplains to align care with patient goals and improve quality of life.
Qualifications include MSN/NP, Maine NP license or eligibility, board certification and BLS, plus prior APP experience.
Qualifications
- Accredited NP program completed with master's level.
- Current Maine NP license or eligible.
- Board certification as NP.
- BLS certification current.
- Experience as an Advanced Practice Provider (APP).
Responsibilities
- Provide inpatient consultation services and ambulatory palliative care with interdisciplinary team.
- Collaborate with patient’s primary care teams to identify goals of care and post-discharge support.
- Document all activities timely and accurately in the EPIC EMR.
- Educate medical students, residents, nurses and APP colleagues on palliative care principles.
- Participate in practice group assignments and meetings.
