Job summary

Hoag Hospital is seeking a Nurse Practitioner for Palliative/Hospice Care in the Home Health setting. You will coordinate care from admission to discharge, perform exams, diagnose and treat, and support discharge planning to ensure patient comfort and quality outcomes.

The role requires a Master’s in Nursing, NP certification, and CA license, with BLS/ACLS and board certification in Hospice & Palliative Medicine preferred. Local travel may be required.

Qualifications

• Master’s degree in Nursing and NP certification.
• 1–2 years of clinical NP experience.
• California RN/NP license in good standing.
• BLS and ACLS certifications.
• National board certification in Hospice & Palliative Medicine.

Responsibilities

• Provide comprehensive NP care in home health for Hoag Hospital.
• Perform history and physical exams, diagnose and treat medical problems.
• Order and interpret diagnostics; manage discharge processes.
• Coordinate multidisciplinary care from admission to discharge.
• Focus on quality and patient experience.
