Job summary

Qualifications

• Requires nursing degree and active RN license in applicable states.
• Experience in palliative care/hospice preferred.
• Knowledge of Medicare regulations and care level criteria.

Responsibilities

• Assess physical, medical, psychological, social and spiritual needs to identify goals and develop a plan of care.
• Collaborate with physicians, discharge planners and staff for transitions of care.
• Provide education and leadership in CCS and staff regarding patient transfers and end-of-life care.
