Qualifications

• RN/APRN licensure in state of practice.
• Nurse Practitioner/CNS with prescriptive authority.
• BCLS licensure.
• Current CPR certification.
• 5+ years of clinical nursing experience.

Responsibilities

• Provide direct patient and family care through palliative care consultations.
• Ensure specialty services for patients with serious illness with prognosis of 1 year or less.
• Deliver ethical, evidence-based palliative medicine practices.
• Conduct comprehensive assessments and plan of care with therapeutic interventions.
• Promote ACHPN certification after 2 years of employment if eligible.
