Qualifications

• Active unrestricted license in the state of practice.
• Current certification as a Nurse Practitioner (APN) in the state of practice.
• National certification by ANCC, NBCHPN, or AANP.
• 5+ years nursing experience.
• Current driver's license and access to a vehicle or reliable transportation.

Responsibilities

• Make face-to-face encounter visits within prescribed time limits and report findings to the Hospice Medical Director.
• Consult with the Hospice Medical Director and coordinate with the referring physician.
• Initiate and coordinate the plan of care; document goals, interventions, and responses.
• Instruct and supervise patient/family in self-care techniques when appropriate.
• Maintain accurate clinical notes and report patient condition to the Administrator and Hospice Medical Director.
