Your Health Organization in Charleston, SC seeks a Nurse Practitioner specialized in Palliative Care to service patients across homes, facilities and telehealth. The role emphasizes evaluating high-risk patients, coordinating care plans, and improving outcomes in a care-team setting.

The position is full-time and salary-based with potential for a monthly bonus, including shifts like 12-hour days on a 7-on/7-off cycle across multiple settings in South Carolina and Georgia.

Qualifications

• Nurse Practitioner license in the state of employment.
• Experience in patient care, preferably in home health, clinic, facility, telehealth, or specialty care settings.
• Strong communication and collaboration skills for interdisciplinary teams.
• Ability to evaluate high-risk patients, develop care plans, and coordinate care activities.
• Commitment to compassionate, patient-centered care and excellence in healthcare delivery.

Responsibilities

• Evaluate high-risk patients with complex medical needs and develop care plans.
• Coordinate care activities with interdisciplinary team to ensure continuity of care.
• Communicate with case managers, coordinators, and others to coordinate patient care.
• Participate in team meetings to discuss patient progress and care plans.
• Document care activities accurately to support plans and compliance.
