Your Health in Walterboro, SC is seeking a Nurse Practitioner specialized in Palliative Care to service patients across Walterboro and surrounding areas. This full-time role includes home visits, facility care, and telehealth, with a possible monthly bonus based on performance.

Responsibilities include evaluating high-risk patients, coordinating care plans, collaborating with the care team, and educating patients and families on treatment options and self-management.

Qualifications

• NP licensure in good standing required.
• Experience in home health, clinic, facility, telehealth or specialty care is preferred.
• Strong communication and collaboration within an interdisciplinary team.

Responsibilities

• Evaluate high-risk patients and develop individualized care plans.
• Coordinate care with interdisciplinary team for continuity of care.
• Communicate with case managers, clinicians, pharmacists and health coaches.
• Participate in team meetings to discuss progress and care coordination.
• Document care management activities accurately for regulatory compliance.
• Educate patients on conditions, treatment plans and self-management.
