Job summary

Your Health Organization is seeking a qualified Nurse Practitioner specializing in Palliative Care to serve patients across Walterboro and surrounding communities. The role includes evaluating high-risk patients, coordinating care plans, and delivering comprehensive care in homes, facilities, and via telehealth.

This is a full-time, salary-based position offering virtual visits from home and in-person care, with the potential for a monthly bonus and a supportive interdisciplinary team.

Qualifications

• Active NP license in good standing.
• Experience in home health, telehealth, or facility care.
• Strong communication and collaboration in interdisciplinary teams.
• Ability to evaluate high-risk patients and develop care plans.
• Willingness to travel between offices and facilities; valid driver's license.

Responsibilities

• Evaluate high-risk patients with complex medical needs and develop individualized care plans.
• Coordinate care activities with interdisciplinary team for continuity and optimal outcomes.
• Communicate regularly with case managers, coordinators, and other professionals to coordinate care.
• Participate in team meetings to discuss patient progress and plans.
• Foster effective communication to support seamless patient management.
• Meet staff efficiency expectations while delivering high-quality care in homes, facilities, telehealth, clinics.
• Document care activities accurately to support plans and compliance.
• Educate patients and families about conditions, treatment options, and self-management.
