Job summary

Your Health Organization in Rock Hill, SC seeks a Palliative Nurse Practitioner to provide home-based patient care within an interdisciplinary team. The role involves evaluating high-risk patients, coordinating care plans, and delivering targeted care management to improve outcomes.

This is a full-time position with monthly bonus opportunities, and requires strong communication skills, license in good standing, and flexibility to travel between offices, facilities, and patients’ homes.

Qualifications

• NP/PA/physician license in the state of employment.
• Experience in home health, clinic, telehealth or ongoing care settings.
• Strong communication and teamwork skills with patients and teams.

Responsibilities

• Evaluate high-risk patients and develop individualized care plans.
• Coordinate activities with interdisciplinary team to ensure continuity of care.
• Communicate with case managers, clinicians, pharmacists, and social workers.
• Participate in team meetings to discuss progress and care coordination.
• Provide patient education and self-management resources.
• Travel for home visits and clinics, documenting care and complying with HIPAA.
