Job summary

Your Health is a leading physician group serving South Carolina and Georgia, seeking a qualified Nurse Practitioner specializing in Palliative Care to service patients throughout the Charleston area. You will deliver care across homes, facilities, telehealth, and clinics, coordinating with an interdisciplinary team.

This is a full-time, salary-based role with monthly bonus opportunities. You will conduct virtual visits from home and in-person visits in homes and facilities; the 12-hour shift

Qualifications

• Nurse Practitioner (NP), Physician Assistant (PA), or Physician with a valid license to practice in the state of employment required.
• Demonstrated experience in providing patient care in home health, clinic, facility, telehealth, or specialty care settings.
• Strong communication, collaboration, interpersonal skills within an interdisciplinary team.
• Ability to evaluate high-risk patients, develop care plans, and coordinate activities.
• Commitment to compassionate, patient-centered care and excellence in healthcare delivery.
• Strong organizational and time management skills, multitasking, and prioritization.
• Ability to read and communicate effectively; strong written and verbal skills; basic computer knowledge.
• Ability to lead and foster cooperative work environment; team player with sound judgment.
• Willingness to travel between offices and facilities; licensed driver with insured vehicle.

Responsibilities

• Evaluate high-risk patients with complex medical needs and develop individualized care plans.
• Coordinate care activities with interdisciplinary team to ensure continuity and outcomes.
• Communicate regularly with case managers, coordinators, social workers, specialists, pharmacists, and health coaches.
• Participate in interdisciplinary team meetings to discuss progress and plans.
• Foster effective communication and collaboration for seamless patient management.
• Meet staff efficiency expectations while providing high-quality care across homes, facilities, telehealth, clinics, and specialties.
• Document care management activities accurately to support care plans and regulatory compliance.
• Provide education and resources to patients regarding conditions, treatment plans, and preventive care.
• Offer guidance to patients and families to enhance understanding of options and participation.
• Adhere to HIPAA, Medicare guidelines, and relevant healthcare regulations.
• Monitor quality measures and identify gaps, implementing corrective actions as needed.
