AccentCare, Inc. is hiring a Full-Time Nurse Practitioner (Palliative Care) at St. Agnes Hospital in Baltimore, MD. The NP will lead patient care plans for chronic and terminal illnesses, conduct assessments, and coordinate symptom management with a supervisory physician.

The role requires NP licensure in the operating state, DEA license, CPR, and willingness to travel to care locations. Competitive salary: $115,000-$125,000; schedule is Mon-Fri, 8-hour shifts.

Qualifications

• Graduated from an approved school of professional nursing.
• One year experience as a Nurse Practitioner is preferred.
• Licensed to practice as a Nurse Practitioner in the state of agency operation.
• Active DEA license in the state where care is being provided.
• Possess and maintain valid CPR certification while employed in a clinical role, preferred.
• Must be a licensed driver who can travel to all business locations.
• Meet the regulations and requirements of the state(s) in which program provides services.

Responsibilities

• Develop and implement clinical plans of care for patients with chronic and terminal illnesses.
• Perform patient assessment, history and physical; recommendations on aggressive symptom-management.
• Monitor patient progress; update the Medical Director, Team Director and team members; discharge planning and patient/family teaching.
• Maintain communication with the supervisory physician(s) and attending physician to review patient records and care plans.
• Initiate communication with attending physicians and other staff to coordinate optimal care and resources.
• Prescribe medication as permitted by the state and/or Board of Nursing; participate in continuum of care for hospice-appropriate patients.
