Job summary

AccentCare is seeking a Nurse Practitioner (Palliative Care) for a full-time position in Los Angeles, CA. The role offers a salary range of $140,000 to $166,000 and a Monday–Friday schedule.

Responsibilities include developing care plans, patient assessments, coordinating with physicians, and prescribing medications as allowed by state law. Active CA NP licensure, DEA, and CPR are required.

Qualifications

• Must be graduated from an approved school of professional nursing.
• One year experience as a Nurse Practitioner is preferred.
• Licensed to practice as a Nurse Practitioner in the state of operation.
• Active DEA license in the state where care is provided.

Responsibilities

• Develop and implement clinical plans of care for patients with chronic and terminal illnesses.
• Perform patient assessment, history and physical; recommendations on symptom-management.
• Monitor patient progress and coordinate with the Medical Director and team.
• Maintain communication with supervisory and attending physicians to review records and care plans.
• Coordinate care with other providers and agencies to optimize patient outcomes.
• Prescribe medications as permitted by the state.
