Job summary

Empatia is seeking a compassionate Palliative Care Nurse Practitioner to join our home-based, interdisciplinary team. You will deliver advanced palliative care, collaborate closely with the Palliative Medical Director, and guide patients and families through treatment.

This role emphasizes autonomous decision-making and comprehensive documentation. The ideal candidate holds an MSN, NP licensure, and at least two years of NP experience in family or adult medicine, with Part B billing experience

Qualifications

Master of Science in Nursing from an accredited school.
Minimum two years’ Nurse Practitioner experience.
Family or Adult Medicine specialty preferred.
Experience with Part B billing.
Ability to work autonomously in an advanced practice role.
Active NP license and state certification.

Responsibilities

Collaborate with the Palliative Medical Director and care team to deliver comprehensive, patient-centered palliative care.
Perform in-depth patient assessments including physical, emotional, and psychosocial evaluations.
Develop and implement individualized care plans with patients and families.
Manage and adjust treatment plans based on patient needs and progress.
Prescribe medications and order diagnostic tests as permitted by state law.
Maintain timely, accurate clinical documentation per standards.
Communicate with physicians, caregivers, and other professionals.
Participate in evening/weekend call rotation as needed.
Facilitate referrals for additional care services.
