Job summary

Hospice of Dubuque is seeking a Palliative Care Nurse Practitioner to act as a primary level care provider under clinical supervision. The role involves delivering palliative care to patients at home or in contracted facilities, with possible in-patient palliative care rotation.

The candidate should be an ARNP with IA licensure (also IL/WI if assigned), DEA and CPR certification, and ideally 2+ years in hospice or palliative care, with strong communication and organizational skills.

Qualifications

• Graduate of an accredited school offering an advanced degree for nurse practitioners (ARNP) — Master’s or Doctorate.
• Certification as an ARNP from appropriate organization.
• Unrestricted license to practice as an advanced nurse practitioner in Iowa (and IL/WI if assigned).
• Current DEA certificate and CPR certification.

Responsibilities

• Provide palliative care visits in patients' residences or contracted facilities.
• Perform palliative consultations and coordinate with patient’s primary care provider.
• Complete documentation for visits and billing; coordinate care with Palliative Care Team.
