UMass Memorial Health is seeking a licensed clinical social worker (LICSW) to join the Social Services team in central Massachusetts. The role focuses on psychosocial assessments, discharge planning, and inter-disciplinary collaboration to support patient care and ensure effective treatment planning.

You will conduct intake evaluations, coordinate daily team meetings, and provide individual and group therapy across settings.

Qualifications

• Master's degree in Social Work.
• Current Massachusetts Licensed Clinical Social Worker (LICSW) licensure.
• One to three years of social work experience.

Responsabilités

• Conduits intake interviews with patients and families to develop psychosocial evaluations.
• Participates in team planning of treatment during initial interview and coordinates daily team meetings.
• Provides psycho-social assessments and treatments, individual and group therapy, including dual diagnosis cases.
• Serves as primary clinician for individual, couples and family therapy.
• Coordinates discharge planning including housing, benefits and treatment needs; plays a key role in case management.
• Acts as liaison with community and government agencies to develop referral networks.
