Job summary

By the Bay Health in the San Francisco Bay Area seeks a Palliative Care Manager to lead its Adult Palliative Care Program and drive growth across the region. You will guide an interdisciplinary team, establish workflows, and strengthen partnerships with healthcare entities to advance compassionate, high-quality care for patients with serious illness.

This RN leader role blends clinical excellence with people leadership and operational execution.

Qualifications

Current, unrestricted California RN license.
Minimum of two years of RN experience in palliative care and/or hospice within the past five years.
Three or more years of supervisory or healthcare leadership experience preferred.
BSN preferred.
Experience leading interdisciplinary healthcare teams.
Strong communication, coaching, and relationship-building skills.
Experience improving clinical workflows, quality, or program performance.

Responsibilities

Lead the day-to-day clinical operations of the Adult Palliative Care Program.
Coach, support, and develop clinical team members while fostering a collaborative and accountable culture.
Partner with the VP of Palliative Care on strategic growth and development of the program.
Design and implement clinical workflows, policies, protocols, and processes for high-quality care and growth.
Coordinate patient referrals and ensure timely assignment, staffing, and continuity of care.
Monitor clinical quality, patient outcomes, documentation, and regulatory compliance.
Collaborate with hospitals and partner organizations to strengthen care coordination.
Identify opportunities to improve patient and family experience during care.
