Job summary

Elevance Health is seeking a Field Advanced Practice Provider to deliver advanced nursing and palliative care in home and client-site settings across Maryland. This field-based role requires flexible travel with some office days, and adheres to the hybrid-work policy.

You will collaborate with physicians and the inter-disciplinary team to develop complex plans of care, perform histories and exams, and educate families.

Qualifications

• MS in Nursing required.
• Current unrestricted NP or PA license in applicable state(s).
• DEA registration or eligibility required.
• CPR certification required.

Responsibilities

• Provides services in home settings to provide care to patients.
• Develops and implements clinical plans of care for adult patients facing chronic and complex conditions.
• Gathers history and physical exam and diagnostics as needed, and develops treatment plans.
• Maintains patient records and coordinates care with interdisciplinary team.
• Prescribes medication as permitted by state authority.
