Job summary

Qualifications
- MS in Nursing required.
- Current unrestricted NP license or PA license in applicable state(s).
- DEA registration or eligibility required.
- CPR certification required.
Responsibilities
- Provides services in home settings to provide care to patients.
- Develops and implements clinical plans of care for adults with chronic/complex conditions.
- Gathers history and physicals and diagnostics; develops treatment plans.
- Coordinates with inter-disciplinary team to meet patient needs.
- Educates patients and families on medications, adherence, and crisis planning.
- Maintains patient records per state regulations and company policies.
- Participates in required continuing education.
- Prescribes medications as permitted by state authority.
