Job summary

Chapters Health System seeks an Advanced Practice Registered Nurse (APRN) to deliver specialized primary care for patients with life-altering illness in SW Florida. The role focuses on comprehensive patient care, documentation, and coordination with interdisciplinary teams to ensure continuity and quality of service.

The ideal candidate holds a Master’s degree in Nursing, an active Florida APRN license, and credentials in Hospice/Palliative care, with BLS certification and DEA license.

Qualifications

• Must possess/ be eligible for Medicare license (Medicaid preferred) as a participating provider.
• Active Florida Advanced Nursing license required.
• Credentialing through ANCC as a NP preferred.
• Certification in Hospice and Palliative nursing preferred.
• Active BLS for healthcare professionals.

Responsibilities

• Provide primary care to patients and families in a professional manner.
• Document patient records meeting good medical practice and legal criteria.
• Provide on-call coverage for patients and families on rotation.
• Coordinate with Hope HealthCare Services and other teams.
