VITAS Healthcare seeks a skilled Nurse Practitioner to provide compassionate, patient-centered hospice care in Gainesville, FL. You will assess patients, develop care plans, and collaborate with physicians to optimize comfort and quality of life.

Join a mission-driven team delivering interdisciplinary care, conducting assessments, prescribing evidence-based treatments, and coordinating with hospice staff to ensure seamless, dignified support for patients and families around the clock.

Qualifications

• Minimum 2 years in hospice, home health or community health in the past 5 years.
• Regularly assist in lifting and transferring patients as required.
• Lifting objects in excess of 20 pounds.
• Specific vision abilities: close/distance vision, focus adjustment.
• Stand or walk for extended periods daily; speak and hear as needed.

Responsibilities

• Records physical findings and formulates plan and prognosis based on patient's condition.
• Discusses cases with physician and other health professionals to prepare care plans.
• Prescribes or recommends drugs or other treatments.
• Monitors and reports progress to supervising physician.
• Collaborates with hospice employees and volunteers as part of an IDT.
