Job summary

CareOne in New Jersey seeks an Advanced Practice Nurse for Bridge Care in the Bergen County area. You will conduct visits to palliative care patients in accordance with local, state and federal regulations and coordinate care with the interdisciplinary team.

You will communicate visit findings to the team, facilitate access to hospice or rehabilitation services, help develop care plans, and maintain accurate patient records to support quality palliative care.

Responsibilities

• Conduct visits to palliative care patients in accordance with local, state and federal regulations.
• Communicate visit findings to designated members of the interdisciplinary team, including the National Director of Palliative Care.
• Facilitate access to appropriate support models of care (i.e., hospice, rehabilitation, on-going aggressive treatment).
• Consult with other members of interdisciplinary team to contribute to the patient's plan of care.
• Prepare and maintain accurate patient records, charts and documents.
• Assist in development of clinical practice guidelines/standards in support of quality palliative care as needed.
• Ensure collaboration with patients, caregivers, medical directors and members of IDT.
