Job summary

CareOne is hiring an Advanced Practice Nurse for Bridge Care (Moorestown/Evesham, NJ). The role involves patient visits, interdisciplinary collaboration, and care plan development in a skilled care setting.

Requires MSN or DNP, and a NJ-licensed Nurse Practitioner with at least 1 year in family/geriatric care. Travel and a valid driver's license are needed; benefits are provided for part-time and full-time staff.

Qualifications

Must have Nurse Practitioner's license in the state of operations.
Must have 1+ years' experience working within Family Practice, General Practice, Geriatric care or related field.
Must have a valid driver's license and a reliable source of transportation.

Responsibilities

Conduct visits to patients in accordance with local, state and federal regulations.
Communicate visit findings to designated members of the interdisciplinary team.
Facilitate access to appropriate support models of care (i.e., rehabilitation, on-going aggressive treatment).
Consult with other members of interdisciplinary team to contribute to the patient's plan of care.
Prepare and maintain accurate patient records, charts and documents.
Assist in development of clinical practice guidelines/standards in support of quality care as needed.
Ensure collaboration with patients, caregivers, medical directors and members of IDT.
