Job summary

CareOne in New Jersey is seeking an Advanced Practice Nurse to join Bridge Care Bergen County. The role involves conducting patient visits, collaborating with the interdisciplinary team, and contributing to the care plan with a patient-centered approach.

The position requires an MSN or DNP and an active NP license in New Jersey, with 1+ year of relevant clinical experience. A valid driver’s license and willingness to travel are essential. Flexible schedule and competitive benefits are offered.

Qualifications

• Must have a Nurse Practitioner license in the state of operations.
• Must have 1+ years' experience in Family Practice, General Practice or Geriatric care.
• Must have a valid driver's license and reliable transportation.
• Must be willing to travel.
• Experience in nursing home, hospital, assisted living or acute rehab is a plus but not required.

Responsibilities

• Conduct visits to patients in accordance with local, state and federal regulations.
• Communicate visit findings to designated members of the interdisciplinary team.
• Facilitate access to appropriate support models of care (i.e., rehabilitation, ongoing aggressive treatment).
• Consult with other members of the interdisciplinary team to contribute to the patient's plan of care.
• Prepare and maintain accurate patient records, charts and documents.
• Assist in development of clinical practice guidelines/standards in support of quality care as needed.
• Ensure collaboration with patients, caregivers, medical directors and members of IDT.

Job Description

Salary Range $125,000 to $135,000 a year

Education

Professional Experience

• Must have a Nurse Practitioner's license in the state of operations
• Must have 1+ years' experience working within Family Practice, General Practice, Geriatric care or related field,
• Must have a valid driver's license and a reliable source of transportation
• Must be willing to travel
• Experience in a nursing home, hospital, assisted living or acute rehab space is a plus but not required.
